Regarding case 69a4b4d9baaf1ff4ca9c9053 — we have a ring-enhancing lesion on CT. What MRI sequences and contrast protocols would you recommend to best differentiate between infectious and neoplastic etiologies? Also, should we get the MRI before or after starting empiric antibiotics?

I would recommend MRI brain with and without gadolinium contrast, including DWI, SWI, and MR spectroscopy sequences. DWI will help differentiate abscess (restricted diffusion) from tumor (usually not restricted). MR spectroscopy can show elevated amino acids in abscess. Get the MRI before antibiotics if clinically stable — antibiotics can alter the enhancement pattern.
